Frequently Asked Questions

TMS Therapy

What is TMS therapy?
TMS (Transcranial Magnetic Stimulation) uses magnetic pulses to stimulate specific mood-regulating brain regions. It's FDA-cleared for depression, OCD and more.
How many TMS sessions do I need?
Standard TMS is 36 sessions over 6–7 weeks, 5 days per week, 20 minutes each. Most patients notice improvement by weeks 3–4.
Is TMS covered by insurance in Arizona?
Yes — most major plans cover TMS when patients have tried 2+ antidepressants without adequate response. We handle all prior authorizations.
Does TMS have side effects?
Mild scalp discomfort during or after a session is the most common. No sedation, no recovery time. You can resume daily activities immediately.

Spravato (Esketamine)

What is Spravato?
Spravato is an FDA-approved nasal spray of esketamine for treatment-resistant depression. Administered in-clinic under REMS-required monitoring by our medical team.
Is Spravato covered by insurance?
Yes — covered by most major commercial plans and Medicare when criteria are met. We handle all prior authorizations in-house.
How long are Spravato sessions?
Each session includes the esketamine nasal spray administration and a 2-hour observation period with our clinical team, as required by REMS.
Do you need a REMS certification?
Yes. Unchained Psychiatry is a REMS-certified Spravato provider — one of a limited number of certified practices in Arizona.

IV Ketamine

What should I expect at my first ketamine infusion?
Sessions last 45–60 min in a monitored clinical environment. Some patients experience mild dissociative effects that resolve quickly. You'll need a ride home.
How fast does IV Ketamine work?
Many patients report relief within hours to days. Full protocol typically involves 6 infusions over 2–3 weeks.
Is IV Ketamine covered by insurance?
Generally self-pay — IV Ketamine is used off-label. Some patients use HSA/FSA funds. Spravato is the insurance-covered alternative.
Can I drive home after a ketamine infusion?
No. You'll need a friend, family member or rideshare to drive you home. Plan for approximately 2 hours total at our clinic.
